An incident response plan should help people make safe decisions under pressure. A long policy that nobody can find or execute is not enough.

1. Treat Response as a Continuous Capability

NIST SP 800-61 Revision 3 organizes incident response across all six NIST Cybersecurity Framework 2.0 functions: Govern, Identify, Protect, Detect, Respond, and Recover. Preparation therefore includes governance, asset knowledge, preventive controls, and detection—not only actions after an alert.

2. Define Scope and Authority

State which systems, cloud accounts, offices, suppliers, and data are covered. Name who can declare an incident, isolate production systems, engage external specialists, approve recovery, notify regulators or customers, and speak publicly.

3. Use Clear Severity Levels

Severity should reflect business impact, data sensitivity, scope, safety, legal obligations, and adversary activity—not just the number of affected hosts.

Define response targets and escalation triggers for each level. Treat these as organizational examples, not universal definitions.

4. Build Small Playbooks

Create focused playbooks for likely scenarios: compromised credentials, malware or ransomware, exposed cloud keys, web compromise, data exfiltration, denial of service, and lost devices. Each playbook should include:

  1. Detection and validation criteria.
  2. Immediate containment options and their business impact.
  3. Evidence to preserve and approved collection methods.
  4. Eradication and recovery prerequisites.
  5. Validation, monitoring, communication, and closure steps.

Use an approved system outside the potentially compromised environment. Preserve original timestamps and record who handled evidence. Legal counsel should define preservation and notification requirements for the applicable jurisdictions and contracts.

6. Plan Communications and Recovery

Predefine secure out-of-band communication when normal email or chat may be affected. Separate technical updates from executive, customer, regulatory, and public communications. Do not speculate; distinguish confirmed facts from working hypotheses.

Recovery requires clean systems, rotated credentials, closed persistence paths, validated backups, heightened monitoring, and explicit business approval. Restore services by priority and verify both security and functionality.

7. Exercise and Improve

Run tabletop exercises and technical recovery tests. After incidents and exercises, record what failed, assign corrective actions, set owners and due dates, and update controls and playbooks. Metrics should improve decisions—such as detection delay, containment time, recovery time, and overdue actions—not reward closing tickets quickly.
